چکیده

Ovarian hyperstimulation syndrome (OHSS) is a lifethreatening iatrogenic complication of in vitro fertilization (IVF). One of the characteristic features of OHSS is increased vascular permeability due to the overproduction of vasoactive mediators from hyperstimulated and enlarged ovaries. Such altered permeability leads to fluid shifts from intravascular to interstitial or third-space compartments. Clinical manifestations, therefore, include hemoconcentration, hypovolemia, decreased renal perfusion, hypotension, electrolyte imbalance, and in severe forms (0.5%–5% of cases), tense ascites. In addition, lower-than-normal levels of plasma immunoglobulin have been detected in patients with severe OHSS, which may predispose patients to serious infections. In this report, we describe a woman with OHSS without previous liver disease who developed a previously unreported infectious complication: spontaneous bacterial peritonitis (SBP).

She received intravenous gonadotropin-releasing hormone (GnRH) along with follicle-stimulating horm...

Spontaneous bacterial peritonitis in extrahepatic portal venous obstruction

Spontaneous bacterial peritonitis is defined by a positive ascitic fluid bacterial culture and an elevated ascitic fluid absolute polymorphonuclear count (≥250 cells/mm3) without an evident intra-abdominal, surgically treatable source of infection. Transient ascites is well documented in patients with extrahepatic portal venous obstruction but spontaneous bacterial peritonitis complicating extr...

Spontaneous bacterial peritonitis in asymptomatic outpatients with cirrhotic ascites.

The prevalence and natural history of spontaneous bacterial peritonitis in asymptomatic patients with ascites secondary to cirrhosis is unknown. From a prospectively recorded database, we reviewed the clinical and laboratory features of all outpatients with cirrhotic ascites undergoing paracentesis between July 1994 and December 2000. The prevalence of spontaneous bacterial peritonitis in the p...
